Do you need users to be able to select more than one option in an item? You can create such an item. It's important to understand what is different about using this feature. In most cases, scoring for such an item will likely not be necessary, although it is possible to do so; however, it is recommended that you contact a Trajecsys representative to assist in setting up scores if this is needed. Therefore, we will focus only on how to create unscored multiple selection items.
Setting up score types: Multiple selection items should always have their own score type. Create a score type with 2 labels: "Yes" and "N/A". No values should be assigned to either label.
Create a new category if you wish (at least one is required for all evaluations), then visit the Evaluation Setup-->Items page. Add multiple select items. For example, let's say that you're building a form to track patient health history. Create a separate item for each choice. For example, enter the first item as "heart disease," select the "Yes N/A" score type you created for the multiple select items, then type the "Group" question or statement, such as "Health History". Save the item, then add a new one -
Item name: Hypertension
Score Type (same as item 1)
Group statement (same as item 1)
...and save.
Item name: Diabetes
Score Type (same as item 1)
Group statement (same as item 1)
...and save.
Make sure that all items have the same group statement and all are organized as items (use Rearrange Items page if necessary), one after the other.
The result will look like this on the form, with checkboxes for each item:
Health History: Heart disease Hypertension Diabetes Kidney Disease Mental illness Cancer
