Select the Templates menu item, then choose to add a new template, or edit an existing one by clicking (at the bottom) or the edit icon . Note that from this page, you can also see which templates are currently visible to evaluators. The magnifying glass is present on many of the setup pages and will allow you to see the existing presentation of the template in evaluator view. Clicking the red "X" will eliminate this template forever...so don't confuse this with making it visible!

Let's look at the settings for the template next.


Enter a template name. This is the title of your evaluation. Use weighted scores only if you want the values of items to be used in your evaluation to differ. If you have 20 questions, each worth 5 points, there's no need to use weighted values; but if you want 5 of those to be worth 50% and the other 15 to be worth 50%, you will need to use weighted score types.

Point Basis is available only if you use weighted scores. You will almost always want your point basis to be 100 (a percentage).

Failing average, if you want one, is the total percentage, using either weighted or unweighted score types, that will flag an evaluation (a red check or "X" will appear under "Completed Evals".

Leave the Edit Lock unchecked unless setup and testing are complete. At that time, check the Edit Lock setting to remove a warning from the evaluator view and to lock score types and other settings from editing. Setting the Edit Lock will assist in preserving the integrity of the data you gather in the evaluation by not allowing changes to the settings established earlier.

Determine the areas, evaluators and subjects. If you haven't established levels for students, your student evaluator option will only be "All students".

While choosing the evaluator is fairly straightforward, choosing the subject correctly can be more complex and may require a good understanding of what information can be gathered and displayed. See the page on choosing evaluators and subjects.

Note that scheduling can be enabled only if the evaluator is a clinical supervisor and the subject is students, or if the evaluator is student and the subject is sites. All other evaluation types are considered unscheduled.

You can choose which user groups, if any, may view results. A number of new options for viewing completed evaluations are now available. Supervisors will now see evaluations completed by site employees without any setting changes if Display to Supervisor is checked; Site Employees will always only see the evaluations with them as subject or evaluator. You may now choose to hide identity from different groups of subjects. Opting to hide from the clinical coordinator and director will result in permanently hiding identities from all program users; this option cannot be revoked.  Please note that sometimes an identity cannot be hidden; for example, if there is one student at a site and the evaluation is displayed to the supervisor, it may be obvious who completed the evaluation, although there may be other ways to obscure the student identity; for example, displaying only after a delayed time period, or only periodically enabling display,  may help.

Some persons can also receive notifications upon approval or disapproval of an evaluation; those persons must be designated in the notifications area.

If you select "Long-Running", the evaluation will not be submitted in the normal way.  Instead, the student will continue to have access to edit the evaluation until he/she is ready to submit the final version.  You will continue to have access to view it during this time.
